How to add /attach a customised enquiry Form to a Group?

The system provides a form that can be customized following steps given below.
● Log in to your account
● Select the Categories option from the main menu
● Click on the "✎" Edit” option of the Category to which customized Enquiry form is to be added
(Refer " How to make a customised Form?")
● In the Form section, select the relevant Form
● Click on Submit and Continue
