How to add/attach a customised Form to the Catalog?

A default form is provided by the service provider with a catalog. We can customise forms and they can be added to the Catalog by following simple steps given below:

● Select the Catalog option from the main menu
● Scroll down to the ‘Forms’ section and select the Form from the drop-down box
(Refer “How to make a customised form?”)
● Click on Submit and Continue
